Discharge planning starts from the moment a person is hospitalized.
The discharge team’s purpose is to:
• provide education, guidance and support
• identify, assess and coordinate the support services that are needed to meet all care needs and ensure a safe living arrangement
• ensure that arrangements are in line with a person’s ability to pay/health insurance options
Although discharge is expected to be timely, by no means should it be rushed. It is a very important for the patient and family to be included in the decision making process related to discharge.
Taking the time you need to communicate your questions to hospital staff will likely result in a successful and safe discharge.
