What questions should I ask during the discharge process?

  • Who is the discharge planner?  Ask this right away.
  • When can I review the discharge plan in its entirety?
  • Have there been any changes to medication?  Are all new medications covered by the health insurance plan?  Is the treating physician aware of all medication changes?
  • Are any new support services required for a safe discharge?  Have they all been arranged through the health insurance plan? How long will they last?  After leaving the hospital, who can be called if there are any problems with these new services?
  • What information (about rest, diet, medication, etc.) is necessary to help manage the condition that caused the hospitalization?
  • What are the warning signs related to the health condition indicating that something may be wrong?  How should one respond to these warning signs?
  • What is the hospital contact number in case of medical difficulties following discharge? Be sure to find out who to call on nights and weekends, too.
  • Who should I follow up with, if I have questions after discharge?
  • Will the treating physician obtain a copy of the updated health record?
