Medicaid: How and when to apply

Medicaid is health insurance for people with low income.  If you believe you are eligible, you can apply for Medicaid at any time.

You must apply in person at your local Department of Social Services office.  In New York City, call the Human Resources Administration at 1-877-472-8411 or click here to go to New York State Department of Health’s Access NY Program web page.

Important! You will need to bring the following documents for the application to be completed:

• your birth certificate
• driver’s license
• pay stubs
• Social Security card
• proof of address
• proof of other insurance if you have it
• information about any bank accounts you have
