Jewish Home Lifecare Establishes Relationship with Senior Whole Health Medicaid Advantage Program

To continue providing our clients with the care and service they need as this requirement takes effect, Jewish Home Lifecare has established a relationship with Senior Whole Health of New York.

Jewish Home Lifecare Establishes Relationship with Senior Whole Health Medicaid Advantage Program

Beginning in 2012 New York State will require Medicaid recipients needing more than 120 days of continuous care in the community to enroll in a managed long term care program.

This requirement will affect Jewish Home Lifecare’s Long Term Home Health Care clients and Adult Day Program clients.

To continue providing our clients with the care and service they need as this requirement takes effect, Jewish Home Lifecare has established a relationship with Senior Whole Health of New York.

Senior Whole Health offers a Medicaid Advantage Program for those eligible for Medicare and Medicaid, who are over 65 years old. The Medicaid Advantage Program includes services covered by both Medicare and Medicaid.

Jewish Home Lifecare Long Term Home Health and Adult Day program clients who choose to enroll with Senior Whole Health will continue to receive the care and services for which they are eligible from their current Jewish Home Lifecare care teams. Clients will also have access to the full range of benefits provided by the Senior Whole Health Medicaid Advantage Program.

In addition to those currently receiving services from Jewish Home Lifecare who are eligible for enrollment, Jewish Home Lifecare will also provide services to new enrollees in the Senior Whole Health program. The Senior Whole Health Medicaid Advantage Program is available to residents of Manhattan, the Bronx, Brooklyn and Queens.
