HealthMonitor Services helps elders stay safe and healthy and remain at home. Using a simple device, daily medication and basic health information can be managed under the guidance of a skilled medical professional. This is particularly useful for elders who live alone, or who have multiple medical conditions.

HealthMonitor Services is a safety-net for elders who live at home.  It helps them comply with their doctor’s medical instructions, which can improve their quality of life.  These services allow for early identification of changes in health and assist in falls prevention, and most importantly, are a vital way to get help in an emergency.  With a health professional monitoring the elder’s status, HealthMonitor Services provide peace of mind to family members who do not live near their elderly parents.

Who can benefit from HealthMonitor Services?

Health Monitor Services products benefit seniors with one or more medical conditions, including high blood pressure, diabetes, congestive heart failure, lung disease or arthritis.  They also enhance ongoing medical care as physicians can see health trends, not just information from the day of a visit.

How does HealthMonitor Services work?

HealthMonitor Services provide easy-to-operate machines to connect to Jewish Home Lifecare’s healthcare providers.

Each morning the device will ask for vital information (symptoms, weight, blood pressure, etc.) and determine if the patient has taken the right medication. This information is then automatically communicated to a Jewish Home Lifecare nurse who can potentially identify problems or areas of concern before they become a health crisis.

If for some reason a client does not respond to the questions, the device will emit a reminder buzz.  If there is still no response, the client will receive a check-in phone call, and if necessary, a visit from a registered nurse.  Knowing that responsive medical help is a phone call away provides independent elders with real security.

For those who find self-administering medication to be confusing, there’s even a model available to manage daily medication.  It can be preloaded by a family member, caregiver or nurse to dispense medication in a plastic cup – exactly according to a doctor’s instructions.

For elders with a history of falls, a Falls Management program with an auto alert will soon be added to the HealthMonitor Services offerings.

What kind of products does HealthMonitor Services offer?

  • Personal Health Monitors are easy to operate. They plug right into the telephone line.  Each morning they record wellness and important health information based on the client’s responses.  This information is relayed to a Jewish Home Lifecare nurse who checks the client’s status seven days a week.
  • Medication Dispensers allow clients to remain independent at home and provide caregivers with peace of mind. They ensure that clients take the proper medications at the proper times from pre-filled dosage cups. The dispenser uses visual and audio alerts to let the client know when it’s time to take medication. If a dose is missed, the system sends a signal to notify caregivers or a Jewish Home Lifecare nurse.
