Adult day program registrants receive:
- Skilled nursing supervision
- Physical, occupational and speech therapies
- Safe, escorted, wheelchair accessible transportation in our own vans
- Nutritional counseling and hot, nutritious meals (Kosher on request)
- Social work services including assistance with Medicaid eligibility
- Exercise groups, celebrations, entertainment and other social and recreational activities
Morning programs are available 7 days a week in Manhattan, the Bronx, and Westchester. Afternoon sessions are available in Manhattan and the Bronx, 5 days a week.
Falls Management
Adult Day Healthcare participants are screened for their risk of falling using Jewish Home Lifecare’s award winning Falls Prevention Plan. The plans are discussed with participants and caregivers. Falls among Day Program participants have been reduced; caregivers report an increased sense of safety for elders in their homes.
Diabetes
Our focus on wellness provides diabetic clients with education and monitoring by specialized nursing staff, in conjunction with nutritional counseling.
Caregivers Support
Social workers work with family members and caregivers to establish appropriate long-term care plans. Education programs are available for caregivers to learn about resources in the community.
Telehealth Technology
Our adult day program incorporates state of the art, award-winning technologies to keep participants well and educated. Ask us more about this!
