Jewish Home Lifecare assembles an interdisciplinary team for Pain Management and Palliative Care for Elders.

A Team Approach to Palliative Care

 The Palliative Care/Pain Relief and Management Program at Jewish Home Lifecare uses an interdisciplinary team approach.  After an initial clinical evaluation and follow-up, the patient’s team is assembled.  Professionals on the team include:

  • Palliative care specialist
  • The patient’s physician
  • Advanced practice nurses
  • Primary nurses
  • Certified nursing assistants
  • Therapists
  • Dietitians
  • Social workers
  • Chaplains
  • Volunteers

The patient and family members are considered key members of the team. Working together, the team determines treatment for pain and non-pain symptoms, and support and education for the family, in times of crisis.
