Geriatric care management
A customized care management program that provides the answers and coordination you need.

Solutions at Home

No matter how much we want to, we just can’t be everywhere at once, especially when it comes to making sure those close to us get the right care. Compound that with all the questions, options and stresses involved in coping with changing life situations, and you can see why Solutions at Home is such a powerful resource.

Solutions at Home is a private pay geriatric care management program from Jewish Home Lifecare that creates a customized solution for each individual, focusing on keeping elders independent and in the comfort of their own homes. Our expert geriatric care managers draw on Jewish Home’s 160-plus years of experience and extensive resources to steer you through the most challenging issues.

Many Options. Many Answers.

Exploring the different options to help a client to remain at home, such as Home Health Care and Adult Day Programs, is just one of the many services we provide. Our geriatric care staff can step into the role of counselor, service provider, adviser or mediator, depending on your needs. We’ll listen and evaluate your family’s circumstances on an individual basis and help with everything, from medical issues or social services, independence or assisted living alternatives, to nutrition and financial planning.

“When we walk in the door to meet each family, we understand that we are facing a lifetime of personal history coming together in a moment of crisis. Perhaps mom has fallen and wants to be home, but the bathroom is on another floor and that just can’t happen. Perhaps the children all live out of state and there are only so many options for dad, who is no longer able to live alone but is resistant to the idea of getting help. We listen with an open agenda and, drawing on our broad resources, help each family make the very best decisions for the patient.” – Patricia L. Mulvey, LMSW, Geriatric Care Manager and Director of Solutions at Home at Jewish Home Lifecare
