Our intensive and supportive therapy is designed for patients who are recovering from surgery or an injury, and no longer need hospitalization, but require comprehensive care to prepare them for returning home. With the patient’s approval, the team can meet family members to coordinate care and plan transition back home. The team can also maintain ongoing contact with the patient’s primary care and specialist physicians. To ensure everything works toward the goal of getting home as quickly as possible, our programs include at least five days of individualized therapy per week.
As each patient progresses, the rehabilitation care team works with caregivers to ease the important transition home. Our rehabilitation does not end when a patient goes home. To make the trip home smooth and effective, each therapy director supervises:
