Jewish Home Lifecare’s Short-Stay Rehabilitation Centers are where you come to work hard, get well and return home. The key to success is not how many hours of rehabilitation you have a day, but how the therapy you receive meets your needs. It’s all about getting you better. You’ll find just this kind of experienced, effective and compassionate rehabilitation at our campuses in the Bronx, Manhattan and Westchester.

The Right Choice

Our intensive and supportive therapy is designed for patients who are recovering from surgery or an injury, and no longer need hospitalization, but require comprehensive care to prepare them for returning home. Our dedicated staff of professionals encourage each patient as they build strength, independence, agility and confidence. Some of the conditions we treat include:

  • Recovery from a stroke or heart attack
  • Hip, knee and shoulder fractures or replacements
  • Major surgical procedures such as post cardiac surgery, amputation and colectomy
  • Effects of cancer treatments

Individual Treatment Plans For Motivated Individuals

Effective rehabilitation relies on support and encouragement. Since every individual’s case is different, we start each relationship by developing a custom care plan. A key part of this is planning for discharge that begins right at admission. This positive, goal oriented approach enhances the teamwork and healing process.Connections

Your Rehabilitation Care Team

Jewish Home Lifecare provides an array of specialists to monitor treatment and progress at every step. Your rehabilitation care team includes:

  • 24-hour, on-site physicians and nursing staff, physical, occupational, and speech and audiology therapists
  • Nutritionists to help recovery and promote wellness
  • Professional social workers to assist with coordinating care and discharge planning
  • Major medical specialists available on-site

With the patient’s approval, the team can meet family members to coordinate care and plan transition back home. The team can also maintain ongoing contact with the patient’s primary care and specialist physicians.  To ensure everything works toward the goal of getting home as quickly as possible, our programs include at least five days of individualized therapy per week.

Discharge Planning & Heading Home

As each patient progresses, the rehabilitation care team works with caregivers to ease the important transition home. Our rehabilitation does not end when a patient goes home. To make the trip home smooth and effective, each therapy director supervises:

  • Ordering adaptive equipment
  • Training patients for ongoing rehabilitation
  • Transportation
  • Transitional home care, if needed

Payment Options

Medicare, Medicaid, and Private Pay options are available. Our financial administrators will work with you to identify, devise, and implement the right plan for you.
